Update: Gas Cylinder Explosion at U of Hawaii – Researcher loses Arm – Update – Analysis Report now Available

For those who have not heard:

A gas cylinder exploded last 6 weeks ago at University of Hawaii biofuels research center.  There was extensive damage and a research technician lost her arm.  This is a sad situation and an unfortunate reminder that safety is paramount when using cylinders.  It appears that this incident could have been avoided as there were some components used that were not intrinsically safe.
